The Foundation will cover the cost of hotel accommodations (room and tax fees) for one Center Medical Director and Center Coordinator at the conference venue, Grand Hyatt Nashville.

Note: Any additional attendees, room nights, or special accommodations must be requested and are subject to approval by the Foundation. Hotel and flight accommodations for additional Centers team members will be the responsibility of the Center and not covered by the Foundation.
Instructions:
In order to reserve a hotel room, you must enter credit card payment information. However, you will not be charged for the room and tax fee as this is covered by the Foundation. But any incidental hotel charges (e.g., mini bar, phone calls, etc.) will be the responsibility of the individual attendee.
